To start the Post Assessment survey you will need to take the token listed above and add it to you SAI home page to make a link to the Post Assessment Survey. Please use the User Name and Password you created for the Pre Assessment Survey and Login. Please do not use the token list above to login with.

Adding your Post Assessment Survey Token

Once logged in you will be on your SAI Home page

Add your Post Assessment Survey Token in the Add Token Field and click on the Add Token button.

Click on the newly added link to take your Post Assement Survey
